If you would rather register online for Walk for Life, just fill out the following form for each registrant. Once finished, click the “Donate” link below the form to pay your registration fee with your debit or credit card. Your registration is not complete until we receive your registration fee.

Registration Fee: $35/person age 10+ (kids 9 and under are free and it is not necessary to register them) OR $150/family* of 5+ (ages 10+) 

*Note for family registrations: Please submit a separate registration form for each member (ages 10+), however, it is not necessary to pay separately. When you click “Donate” to pay with your debit/credit card, it will prompt you to indicate whose registration fee(s) you are paying for.
